Team Registration and Billing Agreement
Complete this form to accept an invitation to an Earth Treks Climbing Team in Maryland. Please do not submit the form until you have received an invitation based on a successful tryout.
Team dues are charged automatically on a monthly basis, September thru June. The first month of billing includes an added start up fee. For a breakdown of fees, team policies, and other important details for parents, please refer to the Parent Handbook and Team Code of Conduct.
Members: Team dues will be added to the normal monthly billing that occurs on the 1st of each month (membership dues, on-account purchases, and team dues will be charged in one lump sum). The billing information presently on the membership account will be used for all charges. Please note that team dues are separate from your membership, and are billed under a separate contract with separate rules, regulations and policies. Non Members: For those participants who are not members, team dues will be processed on the 1st of each month (or the first business day) via credit card. You will need to supply credit card info separately via the secure Change Billing Information form. Indicate in the message box of the billing form that you are providing billing info as part of a team registration and are not members. You do not need a membership to use the form.
